EFT Authorization: Membership dues are transferred via electronic funds on a recurring basis from the credit card or bank information provided. This authorization is to remain effective until Surrey Athletics has collected for all charges assessed in connection with the terms and conditions of Membership Agreement. Monthly payments are due on the same day of each calendar month until 30 days’ written notice of cancellation is served. Membership Agreement shall automatically continue from month to month at the prevailing membership rate, which is subject to change, until 30 days’ written notice of cancellation is served. Member is responsible for all charges billed or incurred prior to cancellation. Membership benefits are exclusive to member and are in no way transferable to anyone else.
Membership Terms/Agreement: Member is liable for all dues for agreed upon Membership Agreement. Failure to attend classes or change of circumstance does not relieve member of the obligation to pay membership dues. Membership benefits are exclusive to the member and are in no way transferable to anyone else. Monthly Membership Agreements (1, 3, 6, and 12 month terms) will be automatically continued month to month past expiration of agreed upon terms until 30 days’ written notice of cancellation is served. Monthly Membership Agreements may be canceled at any time prior to their expiration by paying a cancellation fee equal to 40%+GST of the tuition fee prorated to the remainder of the Agreement. All discounts given will not be considered in the calculation of the cancellation fee. Paid Upfront Memberships are not eligible for refunds and do not auto continue past expiration. If member and/or co-signer wish to cancel a Monthly Membership Agreement on the stated terms set out, they may do so by filling out the Membership Cancellation Form at least 30 days prior to the cancellation date desired.
Membership Holds: A one month membership hold will be granted as many as once every 6 months upon two weeks’ notice. A hold on a month to month membership or past one month every 6 months will be viewed as cancellation and 30 day’s written notice is required. A hold can be requested by filling out the Membership Hold Form.
Class Reservations and No Shows: Class reservations can be made at any time up to 7 days in advance. Class cancellations close 10 hours before the class start time for the 5am and 6:15am classes and 8 hours before the class start time for all other sessions. No Shows will be charged $20.00+tax within 24 hours of event.
EFT Authorization: Full Package dues are due prior to the start of sessions unless discussed with Surrey Athletics. Once the first session is complete, the member is liable for all and any remaining dues owed for the package. This authorization is to remain effective until Surrey Athletics has collected for all charges assessed in connection with the terms and conditions of Personal Training Agreement. Personal Training Package benefits are exclusive to the member and are in no way transferable to anyone else. All unused sessions for any Personal Training Package expire after one year from members first session date and no refunds will be issued. Session
Cancellations: Notice of cancellation is due 24 hours prior to scheduled session time. Notice is required in writing to firstname.lastname@example.org or to contact stream (email, text) used between member and coaching staff providing service. Failure to meet this will result in the session being counted.
